Second dumbest : Climbing out of Vancouver in a Lockeed Constellation, lost number three engine and shut down number four by
mistake. Restarted 4 and greased the landing for a ton of points and my shortest finished FsP flight to date - 7-minutes from brakes off to brakes on. Dumbest : 707-120 out of Denver Stapleton (addon scenery). The Dreaded Wrong Runway... Tried to take off from runway 08L (6900') instead of 08R (10,000'). Ran off the end at 108 knots and hit an inconveniently placed building and ending a streak of 87 consecutive accident-free instant-record FsP flights. Those early Boeing's needed a long run to reach Vr. D'oh.

OHHH I win this!!
I was flying my Learjet 45 from San Franscisco to Vegas. It was visual weather but I selected IFR flight just so I could be vectored to my destination and then chose a visual landing. It was a great flight, perfectly satisfied passengers and all was going well. ATC gave me directions to begin my decent to my destination had me turn onto the track for straight in final. I chose a visual landing and started my set- up (I was still quite a ways away). I got to about 1700 AGL and ATC cleared me to land straight in visual. At this point in time I'd like to mention that I'd been flying the entire thing using AP. When I was cleared to land, I began to disengage a few things on my autopilot and dropped gear and flaps. Then of course, my FO comes on and tells me that my gear isnt locked down...great. I quickly disengadge my flight director and declare emergency. I pull throttle, touch down and brake. Something is wrong, I'm losing runway and not slowing down! My brakes are full and, without really thinking, I put on the emergency brake and set up the speed brakes. I'm starting to slow, but by the end of the runway theres only one thing to do. I slam the left pedal forward a begin to execute a perfect powerslide with my Lear. The plane skids to its side, The weels get closer and closer to the end of the runway..and the ditch happily awaiting my arrival. I hold my breath, keep the skid going and the plane grounds to a halt. I'm off the runway, but managed to avoid the ditch by just a few feet. I open doors, begin to evacuate the passengers and thatnk God that my gear held through it all. It was only then, when I began to shut down that I noticed the 2 lights on the AP panel. The first was the autopilot master switch. The second was the speed hold button, holding my plane at a speedy 250 knots and still trying to propel the plane forward. I shut the engine down and think back. I realise then that when I declared emergency and shut down my AP, I never hit the master, only the flight director. Of course, this means that I missed the speed hold button and touched down at my 250kts cruise speed. Ya, not too bright ![]() Cheers, Kevin

I'd have to say I was on a 5 hour flight in a cessna around the denver area. Perfect flight. I planned the whole route myself. Well I was
rounding hour 4 being bored at 10,000 feet. You know those cessnas can't go much higher. So I was watching a tv show next thing I look over crashed into the side of a 11,000 foot mountain. Lol I always check the mountain height now.

I forgot to set 250 KTS before take off on the auto throttle went to auto throttle after take off and wondered why the airspeed was rapidly heading to
zero! OOPS! MY BAD! ![]()

Forgot to retract speed brakes after decent which caused me to land at -700 fpm....
