A cargo door on a 747 was opened 'uncomanded' some years ago, from Honolulu. The fuesalage was ripped out above the door and
several seats (with passengers) ejected.
Debris took out engines 3&4, though the pilots made a successful emergency landing. (all from Air crash investigations).
Saying something can't happen is always dangerous!
