  • You forgot to close the cabin doors before departure...Passengers in critical condition needed landing as soon as possible but the pilot failed to land in a sufficient short delay and the passengers died during flight.The pilot exceeded the max airspeed for gear extended and gear was damaged. The flaps were extended at excessive airspeed and were damaged.
