is there any way of extending the starting airports on the demo version to italy and malta?

Nope, the demo is limited to the San Francisco area as starting points.
(Well, yes, there is, you can buy it but that's not what you meant I guess )

You will need some traffic program then like Ultimate Traffic or Project AI, Mytraffic etc. etc.
